• What happens to a partially completed visit form?

  Visits that are not submitted after 7 days will be deleted for security reasons.

• How many conditions can I inquire about in a visit?

  Your provider will only evaluate one condition per visit so be very specific with your photographs and description. If you have multiple conditions, please create a visit for each condition.

• How many photos can I upload per visit?

  You can upload up to 5 photos of your condition when you first submit the online visit. After the visit is submitted, you can upload an additional 3 photos if requested by your provider.

• How do I know if my provider has received my inquiry?

  You will be notified by email soon.
